What are the most common types of vascular conditions affecting the legs?

Our physicians have specialized training in the diagnosis and treatment of disease. They have even further specialization in vascular conditions affecting specifically the legs, feet and pelvis.  The most common types of vascular conditions affecting the legs and pelvis are peripheral arterial disease (PAD)and venous insufficiency and varicose veins, but there are other conditions that vascular specialists deal with every day. These include varicose veins in the testes for men, called varicocele, and varicose veins in the pelvis of women, causing a collection of symptoms referred to as Pelvic Congestion Syndrome. Other vascular conditions we treat include May-Thurner Syndrome, geniculate artery embolization for chronic knee pain and uterine fibroids (which are vascular tumors).

What are Vascular Diseases?

Your vascular system is your body’s network of blood vessels. It includes your

• Arteries, which carry oxygen-rich blood from your heart to your tissues and organs

• Veins, which carry the blood and waste products back to your heart

• Capillaries, which are tiny blood vessels that connect your small arteries to your small veins. The walls of the capillaries are thin and leaky, to allow for an exchange of materials between your tissues and blood.

Vascular diseases are conditions which affect your vascular system. They are common and can be serious. Many can be treated without surgery or with endovascular techniques. Some types we treat include:

  • Aneurysm – a bulge or “ballooning” in the wall of an artery

  • Atherosclerosis – a disease in which plaque builds up inside your arteries. Plaque is made up of fat, cholesterol, calcium, and other substances found in the blood.

  • Blood clots, including deep vein thrombosis

  • Varicose veins – swollen, twisted veins that you can see just under the skin

May Thurner Syndrome

Have you had a blood clot in the left leg, or recurrent blood clots without a known cause? Do you notice swelling of the left leg? If so, you may be considered for evaluation of May-Thurner Syndrome, which can be a hidden cause for left leg swelling and DVTs. May-Thurner Syndrome is a condition where the right common iliac artery overlies and compresses the left common iliac vein against the lumbar spine. This causes narrowing of the vein in the pelvic region, sort of like a bottle-neck. The narrowing of the vein makes the blood flow more sluggish and prone to blood clots and leg swelling. This variant has been shown to be present in over 20% of the population.

What Is May-Thurner Syndrome?

The iliac arteries originate from the largest artery in the body, the abdominal aorta. They split at the spine to form the left and right iliac arteries, which feed blood to the pelvis and legs. The right iliac artery is supposed to lie over the left iliac vein, but with this syndrome this artery is compressing the vein. As a result of this compression, the iliac vein narrows and constricts, sometimes scarring. This narrowing results in increased risk of developing blood clots or deep vein thrombosis in that extremity.

Why Do I Have May-Thurner Syndrome?

This condition is congenital but not hereditary and is more common in women than men. It shows up more frequently in young women taking birth control medication, as women age or as they gain weight. Many people may be unaware that they have this disorder, because many may only have some degree of compression of the iliac vein. Only a small number of individuals have severe compression of the iliac vein, resulting in symptoms, namely blood clots in the deep veins of the leg.

How Is May-Thurner Diagnosed?

Diagnosis is achieved by a combination of physical examination and diagnostic testing. Diagnosis may also occur when patients present with symptoms of a deep vein thrombosis (DVT- please see this section of venous conditions) and further testing is completed.

What Are My Treatment Options for May-Thurner Syndrome?

The goal of treatment is to reduce symptoms as well as reduce the risk of further complications.

  • Anticoagulant therapy: These medications thin your blood and prevent the current clot from increasing in size, as well as prevent new clots from forming. These medications do not dissolve the clots that you have. You may be on IV heparin for several days or an injection called Lovenox that is given in the abdomen once daily for 5 to 7 days. You will be started on a medication called warfarin (Coumadin), which is an anticoagulant pill. It can take about 3 days for this medication to have effect, so during that time you may be on both warfarin and heparin. You may be on this medication for up to 6 months, during which time routine blood tests are required to ensure your blood is at the appropriate thinness to prevent clots from forming.
  • Catheter-directed Thrombolytic Therapy: During this procedure, a thin flexible tube called a catheter is inserted into a puncture in the skin and guided into the affected blood vessel of the lung. A thrombolytic drug is injected which dissolves the clot over a period of time. There is a much higher risk of bleeding as well as stroke with this therapy, compared to anticoagulants.
  • Angioplasty and stenting: Once the clot has been removed from the vein, this procedure is performed to widen the compressed vein. A thin flexible tube called a catheter is inserted in a puncture over a vein in the leg. A small balloon at the tip of the catheter is inflated to stretch the iliac vein open, increasing blood flow to the leg. A small metal mesh tube called a stent is inserted through the catheter and guided to the ballooned area of the iliac vein. The stent will help hold the compressed area open and will remain in the vein permanently.
  • Inferior Vena Cava (IVC) filter: During this procedure, a thin flexible tube called a catheter is inserted through a vein in your leg, arm, or neck. The vascular surgeon passes a small special metal filter through the catheter and places it in the inferior vena cava, which is a large vein in your abdomen that carries blood back to the lungs. This filter traps clots that break away from leg veins, preventing them from travelling to your lungs. This is done if the patient cannot tolerate anticoagulation therapy.

Varicocele Embolization for Men

Varicocele Treatment

In most cases, varicoceles treatment is not necessary. Most men with varicoceles do not experience fertility issues. However, if the condition is causing pain, testicular atrophy, or infertility, you can benefit from varicoceles treatment. The treatment helps in sealing off the affected vein and redirecting the blood flow into normal veins. In case of infertility issues, varicoceles treatment helps in improving the quality of the sperm and sperm count. The procedure that we perform at LA Jolla Vein & Vascular on varicocele is percutaneous embolization of varicocele embolization.

The primary goal for varicocele treatment procedures is to help prevent the backflow of blood from the body to the scrotum, helping in cooling off the testes. Surgical procedures will help restrict the blood flow to the veins leading from the testis to the body. However, not all the veins are sealed off. The vassal vein, which is not subject to the same problems as the other varicose veins, is left open to allow blood to leave the testicles after the procedure.

Some of the clear indications that you need varicoceles repair during adolescence include pain, progressive testicular atrophy, and abnormal semen analysis results. While this treatment helps in improving sperm characteristics, it is not clear whether untreated varicoceles can lead to progressive sperm worsening.

Laparoscopic Surgery

Open Surgery

Percutaneous embolism is an advanced procedure performed by a radiologist. The specialist usually makes a small cut into a vein in the groin and inserts a tube. The doctor will use X-ray imaging to guide them to the affected veins and insert a coil or a balloon into it through the tube. The procedure helps in blocking the blood flow to the varicocele, shrinking it gradually. This procedure is also done with general anesthesia.

Catheter-directed Embolization

Catheter-directed embolization is a non-surgical, outpatient treatment performed by an interventional radiologist using imaging to guide catheters or other instruments inside the body. Through mild IV sedation and local anesthesia, patients are relaxed and pain-free during the approximately two-hour procedure.

For the procedure, an interventional radiologist makes a tiny nick in the skin at the groin using local anesthesia, through which a thin catheter (much like a piece of spaghetti) is passed into the femoral vein directly to the testicular vein. The physician then injects contrast dye to provide direct visualization of the veins so he/she can map out exactly where the problem is and where to embolize or block the vein. By using coils, balloons, or particles, the interventional radiologist blocks the blood flow in the vein, which reduces pressure on the varicocele. By embolizing the vein, blood flow is redirected to other healthy pathways. Essentially, the incompetent vein is “shut off” internally by preventing blood flow, accomplishing what the urologist does without surgery.

Efficacy of Embolization for Varicoceles

Embolization is equally effective in improving male infertility and costs about the same as surgical ligation. Pregnancy rates and recurrence rates are comparable to those following surgical varicocelectomy. In one study, sixty percent conceived were treated for infertility.

In another study, sperm concentration improved in 83 percent of patients undergoing embolization compared to 63 percent of those surgically ligated. Patients who underwent both procedures expressed a strong preference for embolization.

Your doctor will make a small 1-inch incision into your scrotum and use a microscope or magnifying glass to see the veins better. The doctor might use local anesthesia to numb the area. General anesthesia can also help you sleep through the procedure.This procedure involves making a much smaller incision and inserting a tube that holds the surgical tools. The surgeon will also use a special camera to see inside you. The procedure is done under general anesthesia to help you sleep through the procedure. The results from the other treatments are similar, only that the incision is small with laparoscopic surgery.The open surgery treatment is usually done on an outpatient basis and using a local or general anesthetic. The surgeon will make a small incision below your groin or abdomen to access the affected vein. Your doctor uses advanced surgical tools such as Doppler ultrasound and a surgical microscope to help guide the procedure. The patient can return to a regular daily routine after two days.

Recovery

In most cases, you can go back to your routine after two days of varicose treatment. However, it is advisable to take it easy. Avoid strenuous activities and exercise for about two weeks.

People who have undergone percutaneous embolization tend to recover faster. While you may need a day or two off from work, you can return to your workout program in 7-10 days. If the treatment procedure is to help with fertility, your doctor will perform an additional test in 3-4 months. You will be able to see improved results in 6-12 months. It is important to note that more than 50 percent of men who have undergone the procedure have restored their fertility. Additionally, these surgical procedures are also crucial for teens as they help in slowing testicular growth.

Genicular Artery Embolization (GAE)

Genicular artery embolization (GAE) is an interventional radiology procedure used to treat knee pain due to osteoarthritis. The procedure minimizes the flow of blood to the knee lining (synovium). Osteoarthritis causes the bones in the knee joint to rub each other, creating friction. This leads to increased flow of blood to the knee due to inflammation. Genicular artery embolization (GAE) is a relatively new and effective minimally invasive procedure that is ideal for people with;

  • Mild to severe knee osteoarthritis
  • People who have not responded to medication, injections, and therapy
  • People who are not read to undergo partial or complete knee replacement surgery

While the procedure does not treat the underlying cartilage damage, it effectively treats the symptoms associated with knee arthritis. Additionally, unlike knee surgery, physical therapy is unnecessary after undergoing the Genicular artery embolization (GAE) procedure.

Aftercare and Recovery

Genicular artery embolization (GAE) is an outpatient procedure performed with moderate ‘twilight’ sedation, meaning that the patient does not need to speed the night in the hospital. The surgeon usually uses x-ray imaging to make an incision in the groin to access the femoral artery. After making the incision, the surgeon will insert a catheter into the femoral artery, which carries the blood to your knee lining.

The catheter injects tiny beads into the arteries to block them and reduces blood flow to the area of inflammation. Reduced blood flow alleviates inflammation, pain, and discomfort associated with knee osteoarthritis (OA). During the procedure, the surgeon usually views the patient’s leg images in real-time, making them precise and effective. The procedure does not take long when compared to other knee surgeries. While the Genicular artery embolization (GAE) length mainly depends on the damage extent, it takes about 1-2 hours to complete the procedure.

Before the procedure, your doctor will brief you on everything you need to know about the procedure. Your doctor will ask you questions regarding your previous surgeries, current/previous medical conditions, and if you are taking any medications. If you take blood-thinning medications or products containing aspirin, the doctor will also issue instructions on discontinuing the medication before the Genicular artery embolization (GAE ) procedure.

You are advised to reach the medical facility early enough to prepare for the procedure. The doctor will ask you to remove the clothes over your knee area and to wear a gown. You will also receive sedation to reduce any discomfort or pain associated with the procedure. The procedure does not require general anesthesia, meaning you will be awake during the procedure. The sedative works to numb the affected area only. It will help if you bring a relative or friend to drive you home after the procedure. It takes time for the sedative to wear out. Of course, you would not want to drive while your leg is partly numb.

Genicular artery embolization (GAE ) procedure, meaning that you will return home the same day. Typically, you will start experiencing pain and discomfort relief in 2-3 weeks as the inflammation of the knee lining reduces. While some patients have reported pain relief almost immediately after the procedure, it is not always the case.

What are varicose veins?

Varicose veins (venous disease) are the twisted, bulging veins just beneath the surface of the skin. Varicose veins are swollen, twisted veins that you can see just under the surface of the skin. These veins usually occur in the legs, but they also can form in other parts of the body. Varicose veins are a common condition with venous disease.

What causes varicose veins?

Varicose veins are caused by leaky vein valves, which allow blood to pool within the veins causing them to stretch and become enlarged. Varicose veins can be an isolated finding, but the majority of the time, they are caused by underlying venous reflux disease. Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Reflux may occur in the deep and/or superficial leg veins.

In our study, led by Dr. Nisha Bunke and published in the Journal of Vascular Ultrasound in 2018, we studied over 1,000 legs with varicose veins. Over 90% of the time, the source of the varicose veins were the great and small saphenous veins. The Great Saphenous Vein (GSV) courses up the middle of the thigh and calf and the small saphenous vein (SSV), which courses along the back of the calf. Normally, there are one-way valves within the leg veins, which help blood flow in one direction: toward the heart. This means blood is traveling against gravity. The calf muscle also helps move blood toward the heart. When vein valves are leaky, blood flows backward (reflux) towards the feet. Blood pools in the lower legs, causing bulging veins at the surface.

Osteoarthritis of the knee:

The most common cause of knee pain is knee osteoarthritis. The pain may come and go, become worse over time, or come accompanied by other symptoms, including knee stiffness. The condition develops due to degeneration of the cartilage. While the cartilage does not contain any nerves, damage or missing cartilage in the knee causes friction between bones and bone tissue changes, leading to pain. 

For example, damaged cartilage can lead to various bone changes such as: 

-Bone Spurs: 

Bone spurs, also known as osteophytes, are abnormal bony growth at the knee joints with damaged cartilage. The joint bones produce the bone spurs to compensate for missing or deteriorated cartilage. Bone spurs have an irregular shape that creates more friction in the knee joint, causing discomfort and pain. 

-Subchondral Bone Sclerosis: 

Due to undistributed weight loads, the tibia and femur surfaces, which lie beneath the cartilage, can change in composition, making it harden. 

-Cysts and Bone Marrow Lesions:

Missing or deteriorated knee cartilage can also lead to cysts development and bone marrow lesions (areas of abnormal swelling). These cysts and lesions lead to knee discomfort and pain.

Osteoarthritis 101

Osteoarthritis is the most common arthritis form that affects millions of people across the globe. While OA can affect any joint, it often damages the knees, hands, spine, and hips. Also known as wear and tear arthritis, the condition develops when the protective cartilage cushioning the ends of the bone in a joint wears away over time. Joints are parts of the body where two or more bones meet. These parts include your knee, hip, hands, backbone (any part of the body that you can bend has a joint).

The ends of the bones in a joint have a smooth, slippery surface referred to as the cartilage. This protective covering allows the bones to rub against each other without friction. Osteoarthritis causes the wear and tear of the cartilage between the bones, making the joint rough. The increased resistance at the joint causes stiffness, pain, swelling, and decreased ability to move.

While various treatments help in managing the OA symptoms, damaged joints are not reversible. Maintaining a healthy weight and staying active c will also help in slowing the disease progression. Although OA can affect both men and women at any age, individuals aged over 45 years are at an increased risk. Based on the research done by the Arthritis Foundation, osteoarthritis affects more than 27 million people in the USA alone, with the knee being the most susceptible part. Research also shows that women are at an increased risk of developing OA than men.

Uterine Fibroids 101

Millions of women around the world are affected by the development of uterine fibroids each year. While it is not a life-threatening condition in itself, the symptoms and potential complications that come with it make it such a pressing medical concern for affected individuals.

As only women will usually possess a uterus, it follows that only women are afflicted by this ailment, which is why gynecologists will often be the first ones to make a definite diagnosis. As with so many medical emergencies, knowing the problem early will greatly increase the chances of beating it, which is why the professionals at La Jolla Vein & Vascular are dedicated to administering reliable, accurate, and entirely effective diagnostics, therapies, and treatments to all clients coming through our doors.

Let’s take a closer look at what exactly we will be dealing with should uterine fibroids turn out to be the issue at hand.

What are uterine fibroids? 

To understand what uterine fibroids are, the first thing one should do is separate the two terms making up the condition. A fibroid is a term to refer to a type of tumor that has ‘muscular’ characteristics. They are also referred to as a leiomyoma and can appear singularly or in formations or in lumps. 

They are known as uterine fibroids since they will appear in the patient’s uterus. Although they are not cancerous in almost all instances, they will range in size from patient to patient, from as small as an apple seed to as large as a grapefruit and beyond, in some unusual instances. 

It is important for all women to be aware of the dangers presented by uterine fibroids, not only owing to the potential harm that the condition might bring about but due to the high rate of prevalence among the female population affected in some way. By the time they are 50 years old, it is estimated between 20 to 80 percent of women will have developed uterine fibroids, with most cases affecting the 40’s to early 50’s. 

Symptoms to look out for: 

Fibroids go undiscovered in most patients due to the lack of symptoms accompanying them, but some patients will experience some symptoms, including:

  • Frequent urination due to pressure being exerted on the patient’s bladder
  • Lower back pain
  • Pain during sexual intercourse
  • Enlargement of the lower abdomen, with severe and extremely rare instances causing the appearance of apparent pregnancy in the patient where none exists
  • Heavy bleeding and/or painful bleeding during periods sometimes resulting in a medically significant depletion of blood in the patient (anemia)
  • Greatly increased risks of complication during pregnancy and delivery, with uterine fibroid patients requiring cesarean sections at six times the average rate.
  • Reproductive health problems such as infertility are very rare outcomes.

Peripheral Arterial Disease – Symptoms

The most common symptom of Peripheral arterial disease is claudication. The fat and cholesterol build up on the artery walls cause a lack of blood flow, causing a condition referred to as ischemia. Ischemia is a condition that results when there is a greater demand for oxygen than the supply.

Claudication is a condition that causes cramping in the legs and buttocks. As mentioned, the pain and clamping flares up when you start to walk and subsides when you rest. Intermittent claudication affects about 50 percent of individuals suffering from peripheral artery disease. Some of the other common symptoms include loss of hair around the ankles, numbness or coldness in your feet.

Pain and cramping while walking may lead to a more sedentary lifestyle, meaning that the individual will burn few calories and add weight. An increase in weight also increases cholesterol, blood pressure, and other heart disease risk factors. The claudication severity often varies from mild discomfort to debilitating pain, making it hard for you to walk or perform other types of physical activities. 

Other common symptoms of the peripheral arterial disease include;

  • In extreme cases, an open wound or ulcer occurs on your toes or feet. These extreme cases lead to non-healing ulcers. The ulcer can progress to gangrene, making it hard for you to walk. In such a scenario, immediate medical attention is necessary.
  • Weakness or numbness in your legs
  • Coldness in your lower feet, especially when you find one foot is colder than the other one.
  • Experiencing pain in your feet or toes while you are resting
  • Sores on the legs, feet, and toes that do not heal
  • Slower toenails growth
  • Change in the color of your feet
  • Erectile dysfunction in men can be treated by prostate arterial embolization (PAE), which also treats benign prostatic hyperplasia (BPH)
  • Weak pulse in your feet or legs
  • Developing shiny skin on your legs
  • Having pain in your arms, especially when writing, knitting, or performing manual tasks

As the peripheral disease progresses, you may start to experience pain even when you are lying down. In extreme cases, the pain becomes intense enough to distract your sleep. Resting your legs by hanging them at your bed edge or walking around the room helps stimulate blood flow, relieving the pain temporarily.

Patient Transformations from venous disease: varicose veins

What are varicose veins (venous disease)?

Varicose veins (venous disease) are the twisted, bulging veins just beneath the surface of the skin. Varicose veins are swollen, twisted veins that you can see just under the surface of the skin. These veins usually occur in the legs, but they also can form in other parts of the body. Varicose veins are a common condition.

What causes varicose veins?

Varicose veins are caused by leaky vein valves, which allow blood to pool within the veins causing them to stretch and become enlarged. Varicose veins can be an isolated finding, but the majority of the time, they are caused by underlying venous reflux disease. Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Reflux may occur in the deep and/or superficial leg veins.

In our study, led by Dr. Nisha Bunke and published in the Journal of Vascular Ultrasound in 2018, we studied over 1,000 legs with varicose veins. Over 90% of the time, the source of the varicose veins were the great and small saphenous veins. The Great Saphenous Vein (GSV) courses up the middle of the thigh and calf and the small saphenous vein (SSV), which courses along the back of the calf. Normally, there are one-way valves within the leg veins, which help blood flow in one direction: toward the heart. This means blood is traveling against gravity. The calf muscle also helps move blood toward the heart. When vein valves are leaky, blood flows backward (reflux) towards the feet. Blood pools in the lower legs, causing bulging veins at the surface.
