Varicose Vein Treatment: Patient Transformations 

What are varicose veins?

They are the twisted, bulging veins just beneath the surface of the skin. These veins are swollen, twisted veins that you can see just under the surface of the skin. These veins usually occur in the legs, but they also can form in other parts of the body. Varicose veins are a common condition.

What causes them?

Varicose veins are caused by leaky vein valves, which allow blood to pool within the veins causing them to stretch and become enlarged. These veins can be an isolated finding, but the majority of the time, they are caused by underlying venous reflux disease. Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Reflux may occur in the deep and/or superficial leg veins.

In our study, led by Dr. Nisha Bunke and published in the Journal of Vascular Ultrasound in 2018, we studied over 1,000 legs with varicose veins. Over 90% of the time, the source of the varicose veins were the great and small saphenous veins. The Great Saphenous Vein (GSV) courses up the middle of the thigh and calf and the small saphenous vein (SSV), which courses along the back of the calf. Normally, there are one-way valves within the leg veins, which help blood flow in one direction: toward the heart. This means blood is traveling against gravity. The calf muscle also helps move blood toward the heart. When vein valves are leaky, blood flows backward (reflux) towards the feet. Blood pools in the lower legs, causing bulging veins at the surface.

What are spider veins?

Spider veins are the fine, thread-like reddish veins at the surface of the skin. These veins are not healthy. Veins are often considered a cosmetic issue, but they can be associated with underlying feeder veins, not visible to the naked eye.  Feeder veins are the ‘blue veins’ also known as ‘reticular veins,’ that give rise to spider veins. Spider veins may also be a sign of underlying venous insufficiency. For example, spider veins located in the inner ankles and inner thighs can indicate an underlying problem with the saphenous vein. As a result, an ultrasound examination may be recommended to identify and effectively treat the underlying source of these veins.

What are the symptoms?

Vein conditions affect people differently. For some, these veins can be a painless cosmetic concern, and for others they may cause symptoms. The most common symptoms of spider veins are burning, throbbing, and localized pain. They can also feel hot and itchy and bleed.

What causes them?

Spider veins in the legs are caused by the same condition that causes varicose veins. Leaky vein valves allow blood to pool within the veins causing them to stretch and become enlarged. These veins on other areas of the body, such as the face, and chest can be caused by sun damage, hormone changes or liver disease. Hormones, such as with pregnancy, birth control or hormone replacement therapy can weaken the vein wall.

Blood clots and vein disease

Deep vein thrombosis (DVT) is a condition in which a blood clot forms in the deep veins located in the leg, thigh, or pelvis. While Deep vein thrombosis (DVT) is common in the lower leg, it can also develop in other body parts, including the arms. So, how do blood clots form in the veins, and is it dangerous? 

To answer this, let us look at how the blood circulates in the body. The blood circulatory system consists of two blood vessels, namely the arteries and veins. The arteries carry blood rich in nutrients and oxygen from the heart, while the veins carry the deoxygenated blood back to the heart. Arteries have thin muscles within their walls that make them capable of withstanding the pumping pressure from the heart. However, the veins have no muscle lining and entirely depend on the muscle movement to take the blood back to the heart. 

How do Blood Clots Form?

The venous system in the legs consists of two main veins, the superficial veins, and deep veins. As the name suggests, deep veins are located deep within the muscles. On the other hand, superficial veins are located just below the skin and are easily visible. Typically, the blood flows from the superficial veins into the deep venous system through perforator veins. Both the perforator and superficial veins have valves that allow blood to flow in one direction. 

However, when blood travels slowly in the veins or pools in the veins, the platelets tend to stick together. While a blood clot (thrombus) in the deep venous system of your leg is not dangerous by itself, it can become life-threatening when it breaks and travels to enter the pulmonary vein. When the blood clot blocks a pulmonary artery, it decreases the amount of oxygen absorbed in the blood, causing a life-threatening condition known as pulmonary embolism. 

Deep vein thrombosis (DVT) is considered a medical emergency. According to the CDC (Center for Disease Control and Prevention), 10 – 30 percent of individuals who develop leg DVT experience life-threatening complications within a month of diagnosis. 

DVT SYMPTOMS

Like other disorders that affect the venous system, some individuals with DVT may not notice any symptoms. However, if the symptoms develop, people with deep vein thrombosis (DVT) may experience the following:

  • Leg pain with walking (muscle cramp or sharp pain) 
  • Sharp pain in the affected limb that starts in the calf 
  • Red or discolored skin on the affected limb 
  • Swelling in the affected limb 
  • Sudden onset leg swelling
  • Enlarged veins 
  • Increased warmth in the swollen, painful region 

Most often, deep vein thrombosis (DVT) affects only one leg, although, on rare occasions, the condition may develop in both legs. If the blood clot (thrombus) breaks and travels up to the lung, a person suffering from pulmonary embolism may have the following:

  • Sudden breathlessness or slow breathing 
  • Rapid breath 
  • Increased heart rate 
  • Chest pain that worsens when breathing deeply

What can I expect with Foam Sclerotherapy?

Ultrasound-Guided Foam Sclerotherapy

Ultrasound-guided foam sclerotherapy works well for treating surface varicose veins and veins that are not visible to the naked eye. The foam medication can be seen on ultrasound, which allows us to administer it precisely and direct it towards nearby varicose veins. Foam sclerotherapy can be safely used for veins that travel close to the skin or nerves. It also travels easily through veins with many twists and turns. 

Foam treatment of varicose veins is not as new as people think. It was originally described as early as 1944.  Foam sclerotherapy is a method for treating varicose veins. It involves injecting a foamed sclerosant medication into unhealthy varicose veins, causing them to eventually dissolve.

 

Colleagues in Spain attracted attention of some vein specialists and interest in the use of foam technology in treating venous insufficiency was reawakened. Administration of foamed sclerosant was reintroduced in the early 1990s by Cabrerra, who summarized a broad experience in 1997. By the 1990’s, broad use of diagnostic ultrasound imaging made it possible to monitor foam distribution with ultrasound scanning. Some 40 years earlier, and before the development of ultrasound scanning, foam had been used in Germany to treat varicose veins.  At that time, foam was made by special syringes and its distribution was assessed by touch, instead of ultrasound scanning.

How many treatments will I need?

The number and frequency of treatments depends on a patient’s anatomy, how well the veins respond to each treatment, and the patient’s treatment goals. The national average is 2 to 5 treatment sessions to achieve 80% improvement. Some larger or resistant veins require two or more treatments to respond completely. 

What should I expect on my treatment days?

You will sign your consent form then change into shorts provided by the office. We will clean your skin with alcohol, then use ultrasound to localize the veins. The foam medication will then be injected into your veins with a fine needle. You will elevate your legs on a wedge pillow for approximately 15 minutes. After your foam sclerotherapy treatment, we will help you into your compression stockings, then you will walk for 30 minutes prior to getting in your car. This is normal for your legs to be achy and tender to the touch after treatment.

WHAT IS VARITHENA?

 

Foam Sclerotherapy

Varithena is a “microfoam” formulation of polidocanol (sclerosing agent) and CO2. The result is a microfoam that has an appearance and consistency similar to white, foamy shave cream. Ultrasound guidance is used to inject Varithena® microfoam into the vein. The microfoam fills the lumen for circumferential contact where it’s designed to displace blood and destroy the endothelial lining efficiently.

Varithena (polidocanol injectable foam)

is a prescription medication used to treat the great saphenous vein system (GSV) and associated varicose veins of the GSV system. Varithena® improves the symptoms of superficial venous incompetence and the appearance of visible varicosities. It is a non-thermal, non-tumescent, nearly painless method of vein treatment. Varithena® may be recommended based on your vein location, size, anatomy and vein tortuosity.

Varithena® is:

  • Effective to treat veins of different sizes above and below the knee
  • Requires no incisions, sedation or general anesthesia
  • Does not require a wire to be inserted along the length of your vein
  • Does not use heat, eliminating the risk of thermal injury

How many treatments will I need?

The number and frequency of treatments depends on a patient’s anatomy, how quickly the veins respond, and the patient’s treatment goals.  There is a limit on the amount of medication we can give you each day to avoid causing side effects.  Some larger or resistant veins may require two treatments to respond completely but most veins respond to Varithena® after one treatment.

You will sign your consent form then change into shorts provided by the office.  We will clean your skin with alcohol, then use ultrasound to localize the veins.  The foam medication will then be injected into your veins with a fine needle.  The foam fills and treats the desired section of the vein.  The diseased vein collapses and the foam is deactivated.  Your legs will be elevated on a comfortable wedge pillow for approximately 15 minutes. After your treatment, we will help you into your compression stockings, then you will walk for 30 minutes prior to getting in your car.

Flying after vein treatment

Patients traveling and flying from different states or countries to seek treatment at La Jolla Vein & Vascular can return to the air within 24-hours after the procedure (although it's understandable if the stay is extended to take in the beautiful sights of San Diego). Although a patient can easily resume most day-to-day activities right after treatment, it's recommended to avoid air travel for the first 24-hours. Once in the air, however, it's important to wear the prescribed compression stockings and work on increasing the circulation in your legs. To do this, please make sure to pump the calf muscles regularly and get up from your chair every two hours. It's the best way to facilitate healing in the air—and it's a sure fire way to avoid economy class syndrome.

There are risks of flying after vein treatment that you need to know about. 

Factors that influence the risks after vein treatment include: 

  • The length of the flight
  • How soon after treatment
  • The type of vein procedure

For flying longer than 4 hours:

If patients have been treated with endovenous laser ablation (EVLA) or ultrasound guided sclerotherapy (UGS), there is an increased risk in development of in-flight deep vein thrombosis (DVT, a leg blood clot). 

If patients have had varicose veins treated, the risk of deep vein thrombosis increases when flying on long-haul flights. 

Although waiting to fly long distances for 3-4 weeks is ideal, sometimes there is no choice. If it cannot be avoided, here are some preventative measures to follow.

1.Wear compression stockings or compression socks: wearing compression helps support consistent blood flow, prevents blood settling in the lower legs because of gravity, and improves lymphatic drainage preventing swelling.

2.Keep moving: Movement is key. It helps contract the calf muscle which helps to move blood around the legs.

3.Hydrate well: Drink water and avoid alcohol (which causes dehydration).

4.Blood thinners: Patients at a particularly high risk of DVT may be prescribed and be taking blood thinners.

5.Most importantly, if you have any questions or concerns to discuss about flying during this time, please contact our office, so you can both be well informed and prepared.

What you need to know about compression stockings

At La Jolla Vein and Vascular, we suggest patients use compression stockings for better vein health. There are a few different types to choose from listed below. But first, knowing the benefits of using compression stockings for your vein health empowers you to decide with your physician which type is best for you. 

Benefits of compression stockings for vein and vascular health: 

  1. Wearing compression stockings or socks places pressure on the veins in the lower legs.
  2. The pressure helps improve blood flow
  3. The pressure helps prevent further spider veins and varicose veins
  4. Compression stockings also relieve leg swelling
  5. Reduces the risk of blood clots in the legs

 

Anti-embolism stockings, also known as TED hoses, are designed specifically for non-mobile patients or those confined to a bed. These are the white stockings used for hospital patients. They are low-cost temporary solutions commonly used for patients in nursing homes and post-surgery to prevent deep vein thrombosis (DVT).

 

Graduated compression therapy provides external support to the vein walls and works in conjunction with the calf muscle pump. Compression stockings improve circulation while helping to minimize and control leg and ankle swelling.

However, for ambulatory (walking patients who are not bed-bound), TED hoses do not offer sufficient support to counter the effects of gravity.  They have not graduated compression and only offer about 8-18mmhg compression. TED hose does not help the symptoms of venous disease and varicose veins. TED hose cannot be used for vein treatment and should not be used for daily support in walking patients.

In comparison, graduated compression stockings are medically therapeutic and designed for people who are mobile. Graduated compression means that they are tightest around the ankle and gradually ease as they go up. These can help reduce the risk of DVT in patients who travel by plane or car and reduce symptoms such as leg swelling (edema), aching, heaviness, fatigue, pain from varicose veins, and useful for pregnant women to reduce pain from varicose veins.

Improving your vein health through exercise

Physical activity and exercise provides a wide range of benefits for vascular health and may help to avoid evolution of mild cases of varicose veins. In fact, exercise is considered to be a fundamental element in improving the symptomatology of patients affected by varicose veins. A sedentary person diagnosed with this medical condition has a much higher risk of worsening the symptoms when compared with an active person with the same condition. This is a result of blood pooling in the veins, causing an increase in venous hypertension and symptoms.

The main goal of exercise in regards to varicose veins is to contract and move the muscles of the leg, helping to pump the blood upwards, avoiding edema or retention of liquid in the ankles. With this in mind, the recommended exercises are those with aerobic characteristics rather than those with anaerobic ones. Through this physical activity, the pressure in the veins is improved, as well as the resulting symptomatology.

Therefore, any exercise that involves moving the lower limbs and promote cyclical muscle contractions is advisable, including stand on tiptoes, move the toes, perform foot bending and rotation, do pedaling movements, among others. These can be easily performed throughout the day without the necessity to go out and exercise, and are especially useful during work hours or while doing daily tasks at home.

When walking or running, pressure is exerted on the sole of the foot, which causes the circulation to be activated from the bottom up, while the constant contraction of the muscles during cycling causes the same effect, but without the presence of high impact, an important factor for those with joint issues.

Swimming is one of the best exercises to practice when affected by varicose veins. The double effect of the water and the movement of the lower limbs cause an incredible increase in blood circulation. This is helpful also for patients who have significant symptoms related to the effects of gravity, like standing.

Other disciplines like yoga, pilates, or rhythmic gymnastics also help stimulate circulation by mobilizing the accumulated blood in the thighs, while relaxing the whole body.

Hiking is a great activity for using the calf-muscle pump. However, in warm weather, symptoms of varicose veins worsen. To get the maximal benefit of exercise and reducing symptoms, outdoor exercising when the weather is cooler, like in the morning is advised.
