Endovenous Laser Ablation

VenaCure is an endovenous laser ablation (EVLA) procedure for backwards flow (or “reflux”) in your saphenous vein(s).

Laser is also referred to as endovenous laser ablation therapy (EVLT). The great and small saphenous veins are the two main superficial veins of the leg. They run along the inner leg and the back of the leg, respectively.

This minimally invasive procedure can be performed in the office in less than an hour and patients return to their usual level of activity the same day.

How does the endovenous treatment work for reflux?

The skin is numbed with lidocaine, then a tiny wire and the laser catheter are inserted into the vein. The catheter delivers laser energy to the vein wall, causing it to seal shut. The remaining healthy veins continue to bring blood back to the heart.

WHAT SHOULD I EXPECT ON THE DAY OF TREATMENT FOR REFLUX?

The procedure is performed with local anesthesia, but many patients elect to use a mild oral sedative (Valium), which is taken after checking in and completing all paperwork.

You will change into a gown and leave underwear on. Depending on the vein to be treated, you will lay on your back or on your belly.  We do our best to make special accommodations (for example, if you cannot lie flat or cannot bend a knee very well) with body positioning and using pillows.  We will do our best to make you comfortable. Then, we will give you the option of watching a movie on Netflix or listening to music. Once you are comfortable, your leg (s) will be prepped with a cleansing solution for the sterile procedure. The doctor will perform an ultrasound to map the vein (s) to be treated.

Then, a numbing agent (lidocaine) will be injected into the skin. In the numb area of the skin, a tiny puncture is made to pass the laser catheter under ultrasound guidance.  Your doctor will then use a needle to administer a combination of cool saline and local anesthetic around the vein.  This solution numbs the vein and insulates it from the surrounding tissue.  Laser energy will then be used to painlessly treat the vein.

Once your vein has been treated, we will help you into your compression stocking, which you will wear for 72 hours continuously. You will walk for 30 minutes prior to getting in your car.

RFA for perforator vein reflux

Radiofrequency perforator ablation (RFA) is performed on perforator veins, short connections between your deep and superficial veins. Perforator veins can suffer the same sort of valve damage and retrograde blood flow (venous reflux) as other veins. Most patients with vein disease who experience skin changes, (that also may result in leg ulcers if left untreated) have reflux in perforator veins near their ulcers, as well as in the superficial or deep veins.

What are Perforator Veins?

Perforator veins serve as connections between the two networks of veins in the extremities, the superficial venous system and the deep venous system. Perforators connect the two parallel systems, and normally drain blood from the superficial veins to the deep veins as part of the process of returning oxygen-depleted blood to the heart.

Perforator veins have one-way valves designed to prevent backflow of blood down towards the superficial veins. When those valves no longer function properly and reflux occurs, the buildup of blood and pressure can cause not only the superficial veins but the perforators themselves to become incompetent. Perforator veins in the lower leg and ankle are particularly vulnerable to distention and incompetence, and the resultant circulatory problems create an increased likelihood of edema, skin discoloration, dermatitis and skin ulcers in the immediate area.

Your doctor may have recommended RFA treatment of your perforator vein(s) because it is contributing to:

  • Venous leg ulcers
  • Pre-ulcerative skin changes
  • Persistent or recurrent varicose veins
  • Leg pain
  • Leg swelling

What are the treatment options for PVCs? Available treatment options for PVCs include pain-relieving progestin hormone drugs, ovarian function-blocking Gonadotropin-releasing hormones, vein embolization, and surgery to rid of damaged veins, uterus, and ovaries. Customarily, your doctor will start you on the medications first to relieve pain. If the medications fail, then they may advise on the next step, usually the invasive options. Vein embolization is the most commonly used procedure. Here’s a look at what it involves:

Pelvic Venous Embolization

Treating PVCS diseases like pelvic and labial varicose veins requires resolving the backward flow of blood in the ovarian and pelvic veins through embolization. Embolization is an outpatient procedure that uses catheter technology to access the venous system to the problematic ovarian veins. The procedure is painless and requires no major incisions. The physician embolizes each problem vein (blocking blood flow) with vascular plugs or coils. Doing so permanently seals off the varicose veins, diverting blood flow up through healthy veins. The physician will also deliver sclerosing medication to lower the pressure in the pelvic veins and relieve the pain. Once blood can flow smoothly through healthy veins in the pelvic and labial area, the pain disappears. Patients can usually return to work and light activities the day following the procedure and see a full recovery in about one week; at least 7 in 10 women who undergo the procedure report feeling better. This procedure is usually performed by a trained interventional radiologist in the X-ray department.

 

Sclerotherapy

Sclerotherapy is a minimally invasive procedure that injects sclerosing solutions into the veins. It is used to treat spider, reticular and varicose veins. It also blocks the unsightly or non-functioning veins to gradually improve appearance. This generally involves multiple treatments over the course of several months.

 

Radiofrequency Ablation (RFA)

Radiofrequency Ablation is a minimally invasive, image-guided procedure used to treat varicose veins. An RFA utilizes high-frequency radio waves directed through a thin tube to create intense heat within the varicose vein. This closes up the problem vessel so blood cannot flow through it, redirecting blood flow into healthy veins to allow the blood to return to the heart. Eliminating the unhealthy vein reduces the vein bulging and minimizes leg pain and swelling. It is performed as an outpatient procedure with minimal recovery and discomfort.

 

When Should I Contact My Doctor?

Some women have enlarged veins with no symptoms; others have enlarged veins that cause pains and aches. In most cases, these don’t require a medical emergency. The symptoms should reduce as you head into menopause. However, in case of chronic, unbearable pain, nothing is stopping you from seeking medical help. In fact, see your healthcare giver right away for help.

PVC: Most people know varicose veins mostly affect the feet and legs. But varicose veins can also occur in the pelvis causing chronic pelvic pain in women; this condition is known as Pelvic Venous Congestion Syndrome (PVCs) or ovarian vein reflux. Herein is a look at the condition, the risk factors, causes, symptoms, and treatment options. Read on to find out more.

What is pelvic venous congestion syndrome (PVCs)?

 

Pelvic Venous congestion is the enlargement of blood vessels in the pelvis due to faulty vein valves in the lower abdomen. See, veins have valves to guide blood flow towards the heart. However, these valves become faulty in some cases due to damage or other problems causing blood to flow backward. When this happens in the lower abdomen, blood builds up in the area, causing the veins to enlarge and change shape.

Therefore, PVCs are varicose veins in your pelvis. This engorgement or congestion of blood vessels in the lower abdomen can cause chronic, unbearable pain. It affects at least 1 in three women at some point during their lifetime. Chronic PVCs last more than six months. It is not associated with period pain at all. PVCs are common among women who’ve given birth more than once.

Symptoms to Look Out For

Chronic PVCs last longer than half a year. PVCs are commonly experienced for the first time during or following pregnancy. It is characterized by a heavy aching feeling that may get worse as pregnancy progresses. In most cases, you feel the pain on the left side only. But at other times, you may feel the pain on both the left and right sides. PVCs pain is usually worse during the evenings.

Certain factors also aggravate PVCs pain; these include:

  • When you change posture
  • Sex
  • Standing for extended periods
  • Walking
  • Menstrual period

Apart from pelvis pain, other PVCs symptoms include:

  • Pain during or after intercourse
  • Sudden urge to urinate
  • Lower back pain
  • Irritable bowel syndrome (diarrhea and constant abdominal pain accompanied with constipation)
  • Deep dyspareunia – pain during intercourse
  • Feeling one’s legs fuller
  • Engorged and distorted veins around the vagina, vulva, inner thigh, buttocks, and sometimes down the legs

Blood clots and vein disease

Deep vein thrombosis (DVT) is a condition in which a blood clot forms in the deep veins located in the leg, thigh, or pelvis. While Deep vein thrombosis (DVT) is common in the lower leg, it can also develop in other body parts, including the arms. So, how do blood clots form in the veins, and is it dangerous? 

To answer this, let us look at how the blood circulates in the body. The blood circulatory system consists of two blood vessels, namely the arteries and veins. The arteries carry blood rich in nutrients and oxygen from the heart, while the veins carry the deoxygenated blood back to the heart. Arteries have thin muscles within their walls that make them capable of withstanding the pumping pressure from the heart. However, the veins have no muscle lining and entirely depend on the muscle movement to take the blood back to the heart. 

How do Blood Clots Form?

The venous system in the legs consists of two main veins, the superficial veins, and deep veins. As the name suggests, deep veins are located deep within the muscles. On the other hand, superficial veins are located just below the skin and are easily visible. Typically, the blood flows from the superficial veins into the deep venous system through perforator veins. Both the perforator and superficial veins have valves that allow blood to flow in one direction. 

However, when blood travels slowly in the veins or pools in the veins, the platelets tend to stick together. While a blood clot (thrombus) in the deep venous system of your leg is not dangerous by itself, it can become life-threatening when it breaks and travels to enter the pulmonary vein. When the blood clot blocks a pulmonary artery, it decreases the amount of oxygen absorbed in the blood, causing a life-threatening condition known as pulmonary embolism. 

Deep vein thrombosis (DVT) is considered a medical emergency. According to the CDC (Center for Disease Control and Prevention), 10 – 30 percent of individuals who develop leg DVT experience life-threatening complications within a month of diagnosis. 

DVT SYMPTOMS

Like other disorders that affect the venous system, some individuals with DVT may not notice any symptoms. However, if the symptoms develop, people with deep vein thrombosis (DVT) may experience the following:

  • Leg pain with walking (muscle cramp or sharp pain) 
  • Sharp pain in the affected limb that starts in the calf 
  • Red or discolored skin on the affected limb 
  • Swelling in the affected limb 
  • Sudden onset leg swelling
  • Enlarged veins 
  • Increased warmth in the swollen, painful region 

Most often, deep vein thrombosis (DVT) affects only one leg, although, on rare occasions, the condition may develop in both legs. If the blood clot (thrombus) breaks and travels up to the lung, a person suffering from pulmonary embolism may have the following:

  • Sudden breathlessness or slow breathing 
  • Rapid breath 
  • Increased heart rate 
  • Chest pain that worsens when breathing deeply

Concerned about Leg Pain?

We evaluate and treat leg pain that is related to poor circulation, varicose veins and blood clots. These are all vascular conditions. However, there are numerous causes of non-vascular leg pain including injuries in the joints, bones, muscles or ligaments. Arthritis can also cause leg pain. Low back pain can radiate to the legs and also cause pain. It is always important to have a thorough evaluation by your doctor to determine the correct cause.

  1. Blood Clots: 

Leg pain caused by blood clots can vary in how it presents. Not all blood clots will cause pain. Blood clots in the deep veins, called deep vein thrombosis, (DVT) can cause a sudden onset of swelling in the leg, pain, and sometimes redness and warmth. It usually affects one leg instead of both. It may be difficult to walk.  A blood clot in a superficial vein, called superficial vein thrombosis, often occurs in a varicose vein. The vein can feel hard, painful, lumpy, or you may feel a rope-like knot just beneath the skin. The skin may appear red and hot.

 

  1. Venous Reflux Disease:

Leg pain caused by venous reflux disease often presents as leg pain, heaviness, fatigue, that tends to get worse throughout the day and with standing. It may be worse in hot weather. Women may experience worsening symptoms during menstruation from hormonal influence. Night-time symptoms of leg cramping and restlessness are common. There may or may not be visible signs of venous disease, as venous reflux usually starts in veins that are not visible to the naked eye before becoming visibly apparent. Signs of vein disease include varicose veins (bulging, twisted, enlarged veins and the surface of the skin), spider veins, ankle swelling, and long term venous reflux, called chronic venous insufficiency can cause the skin to darken around the ankles and ulcerate.

  1. PAD

Poor arterial circulation in the legs is referred to as peripheral arterial disease (PAD). PAD has a different presentation than venous disease. Leg pain is often with walking and can cause cramping most frequently in the calf muscles but also can occur in the thighs or buttocks. The cramping stops usually with rest. Signs may include poor perfusion to the toes causing paleness and thickened toe-nails. Pulses at the feet may be absent.

Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Venous reflux disease refers to ‘leaky valves’ in the veins of the legs. Reflux may occur in the deep and/or superficial leg veins. The deep veins are those within the muscle; they bring at least 80-90% of the blood from the legs back to the heart. The superficial veins are outside of the muscle and under the skin.

The main superficial veins are the Great Saphenous Vein that courses up the middle of the thigh and calf and the small saphenous vein, which courses up the back of the calf. Normally, there are one-way valves within the leg veins, which help blood flow in one direction: toward the heart. This means blood is traveling against gravity.

The calf muscle also helps move blood toward the heart. When vein valves are leaky, blood flows backward (reflux) towards the feet. Blood pools in the lower legs, causing bulging veins at the surface. Symptoms include leg heaviness, leg fatigue, leg pain, ankle swelling, phlebitis (inflamed and painful veins) , restless legs at night, and night cramps.

Venous reflux disease is progressive and worsens over time. Skin changes may also develop, including darkening of the skin around the ankles. The darkening of the skin is sometimes referred to as venous stasis skin changes. The skin can become dry and itchy (venous eczema). Eventually, the skin can break down causing a wound, called a venous leg ulcer.

Step 1: The Underlying Problem

The first step is to treat the underlying problem, the venous reflux. The specific pattern of venous reflux was detected by ultrasound. Venous reflux usually starts in the saphenous veins. The saphenous veins are most effectively treated with vein ablation procedures. This involves placing a small catheter within the vein and using heat or a solution to produce injury and eventual closure of the vein. The most commonly used treatments for the saphenous veins are radiofrequency ablation (RFA), laser ablation, mechanico-chemical ablation (MOCA or Clarivein), and in some cases Varithena Foam. The treatment Step recommendation is customized, based on where reflux is present and other clinical factors.

Step 2: Varicose Veins

After the underlying saphenous vein reflux is corrected, the bulging veins (varicose veins) can be treated by injecting a foamed medication that will cause them to scar and eventually dissolve (foam sclerotherapy), or to remove them using tiny incisions. The most common method is foam sclerotherapy. This is also known as ultrasound-guided foam sclerotherapy (UGFS).

Step 3: Spider Veins

Spider vein treatment is always considered a cosmetic procedure. If improved cosmesis is a goal of treatment, make sure to discuss this with your doctor and plan on having cosmetic treatment as the last step. Spider vein treatment is accomplished by sclerotherapy, which is an injection of a liquid medication into the spider veins. Ask your provider for an estimate about how many treatment sessions/ vials are necessary to get the results you want.

In addition to your physical exam findings and medical history, an ultrasound examination is an important tool in the assessment of venous disease. Not all venous disease is visible to the naked eye, and it usually arises from veins that are beneath the surface of the skin, only visible by ultrasound technology.

The ultrasound allows us to see if the valves are leaky; it can detect the direction of blood flow and also detects blockages in the veins, for example from blood clots or scars within the veins from previous clots.  The ultrasound will determine exactly which veins are ‘bad’ or incompetent.  Reflux may be detected in the deep veins (within the muscle), the great and small saphenous veins, and/or branches of the saphenous veins.  This will help determine the treatment plan.

Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Venous reflux disease refers to ‘leaky valves’ in the veins of the legs. Reflux may occur in the deep and/or superficial leg veins. The deep veins are those within the muscle; they bring at least 80-90% of the blood from the legs back to the heart. The superficial veins are outside of the muscle and under the skin.

The main superficial veins are the Great Saphenous Vein that courses up the middle of the thigh and calf and the small saphenous vein, which courses up the back of the calf. Normally, there are one-way valves within the leg veins, which help blood flow in one direction: toward the heart. This means blood is traveling against gravity.

The calf muscle also helps move blood toward the heart. When vein valves are leaky, blood flows backward (reflux) towards the feet. Blood pools in the lower legs, causing bulging veins at the surface. Symptoms include leg heaviness, leg fatigue, leg pain, ankle swelling, phlebitis (inflamed and painful veins), restless legs at night, and night cramps.

Venous reflux disease is progressive and worsens over time. Skin changes may also develop, including darkening of the skin around the ankles. The darkening of the skin is sometimes referred to as venous stasis skin changes. The skin can become dry and itchy (venous eczema). Eventually, the skin can break down causing a wound, called a venous leg ulcer.

Varicose Veins and Venous Reflux Disease

Varicose veins are the twisted, bulging veins just beneath the surface of the skin. Varicose veins are swollen, twisted veins that you can see just under the surface of the skin. These veins usually occur in the legs, but they also can form in other parts of the body. Varicose veins are a common condition.

What causes varicose veins?

Varicose veins are caused by leaky vein valves, which allow blood to pool within the veins causing them to stretch and become enlarged. Varicose veins can be an isolated finding, but the majority of the time, they are caused by underlying venous reflux disease. Venous reflux disease is also known as venous stasis, venous insufficiency or venous incompetence. Reflux may occur in the deep and/or superficial leg veins.

In our study, led by Dr. Nisha Bunke and published in the Journal of Vascular Ultrasound in 2018, we studied over 1,000 legs with varicose veins. Over 90% of the time, the source of the varicose veins were the great and small saphenous veins. The Great Saphenous Vein (GSV) courses up the middle of the thigh and calf and the small saphenous vein (SSV), which courses along the back of the calf.
